What causes an accident?  Is it lack of discipline, unprofessional behavior, lack of ethics, fatigue, negligence, miscommunication, or a combination of those factors?

This module will present a case study of Colgan Air 3407 crash to observe which factors contributed to the accident and if it could have happened to anyone.  Could this have been avoided with knowledge-based training and personal accountability?

We don’t get to pick a day when our knowledge, experience, and skills are tested. Life does not give advance warning for challenging situations. We must make it a personal responsibility to prepare, study, and seek what we don’t know. 

This presentation will give you the insight into Safety Standdown principles and why simply adhering to the regulations may not be enough to keep us safe.
